Operation Brain Health Support Line

Are you or a loved one suffering from lingering symptoms and not sure where to turn to next?

Here at Project Enlist Canada, our goal is to help you throughout every stage your head injury recovery. We know that navigating the healthcare system can be a daunting task, which is why we have created this resource to help you and your loved ones with this process. The Operation Brain Health Support Line is a program operated by the CLF HelpLine to provide support for Canadian current and former serving members and their families.

Our dedicated coordinator will work with you to give you guidance finding a doctor, find the appropriate intake/referral forms, and answer any questions you may have.

We are not a medical team and cannot give you medical recommendations, but what we can do is help you gain access to the care you need during whatever stage of your recovery you may be in.

Patient Information
This section asks about the patient and their history of brain trauma exposure. Complete this section to the best of your ability regarding your knowledge of the patient's history so CLF HelpLine case managers can respond with the best possible resources. Some responses are required to submit the form.
